How to Join the Cynon Vale Medical Practice

Joining Cynon Vale Medical Practice is easy. Providing you live within our practice area (generally a CF45 postcode, but see map below), simply pick up an official registration form at our reception desk, and, once completed, hand it back to us. We will then arrange for all your medical records to be transferred from your current surgery.

All patients are given an appointment with the nurse for a short medical check up and with a doctor if on medication. You are asked to bring along a urine sample with you on this visit.

Once you have been given an appointment for your new patient check you may wish to download the New Patient questionnaire, complete it and bring it to your appointment

The practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation or appearance, disability or medical condition.


Accountable / Named GP for All Patients

All patients registered at Cynon Vale Medical Practice have a named doctor who has overall responsibility for your care and support. If you choose to see another doctor at the surgery you are entirely free to go on doing so exactly as before.

Disabled Patient Facilities

This surgery is pleased to announce that we have improved access for disabled patients. This access is to the side of the building. However, the practice will continue to provide house visits where appropriate.
